If you’re a new North Star Counseling client, please complete the following two forms and scan and email them to your therapist or bring them to your first session. You may view then print or download them here or download them below.
OPTIONAL NSC FORMS
(Optional) You may print and bring/send this form to authorize your therapist to release information to a third party.
(Optional) If you plan to have a third party (family member, church, other party) pay for all or part of the fee for service, please print this form and have them note their commitment amount and contact information then sign and date it.
